山东啃下公立医院综改“硬骨头”   编者按   改革正呈现出全面发力、多点突破、蹄疾步稳、纵深推进的良好态势。山东坚决贯彻落实中央决策部署,迎难而上、锐意攻坚,在农村、教育、医疗、科技、金融等领域的改革都走在了全国前列。本报今起推出“改革攻坚·新突破新亮点”专栏,系统梳理改革举措,盘点成效,总结经验,以期在全社会增强良好改革预期,凝聚起推进改革的强大动力。敬请关注。   打破编制“紧箍”,38.2万人“备案”管理;医疗服务价格放开,取消药品加成58亿元   山东啃下公立医院综改“硬骨头”   □ 本报记者 王凯 李振 本报通讯员 高晖   一场没有编制的公立医院招聘引来热捧。9月14日,聊城市卫计委公布了2016年市直公立医院公开招聘备案制工作人员面试入围名单,7家公立医院参与招聘,入围人员达493人。打破人事编制的“紧箍”,释放医生人力资源,山东公立医院人事编制改革走在全国前列。   5月,省编办和省人社厅公布了《关于公立医院人员控制总量备案有关事项的通知》,自2016年起,我省不再安排公立医院用编进人计划,由医院根据业务水平、类型特点、床位数、门诊量等确定人员控制总量,向同级机构编制部门报备。   院长有了“用人权”。在岗位设置方面,医院可以自主拟定岗位设置方案,对医护人员、管理人员和后勤人员实行分类管理;在用人方面,允许医院根据业务需要,自主聘用已经取得专业技术职称的医护人员,并实行竞聘上岗、按岗聘用、合同管理,变固定用人为合同用人,变身份管理为岗位管理;在养老保险方面,人员控制总量内的所有医生,以及其他具有中级职称以上的专业技术人员,都可以参加事业单位养老保险,并建立职业年金制度。   编制人事制度更加合理,用人机制更加灵活,收入分配机制的激励作用更加积极有效,医务人员价值凸显。在全国范围内,山东公立医院人事编制管理较早实现了突破。目前,全省已备案人员总量为38.2万名,其中省属22家医疗机构3.9万名,各市440余家医疗机构34.3万名。   不止是人事编制改革,山东接连啃下医疗服务价格调整、分级诊疗制度建设、医院法人治理结构等“硬骨头”,逐步构建山东医疗卫生体系的新格局。   医疗服务价格大胆尝试放开。对市场竞争比较充分、个性化需求比较强的医疗服务项目,实行市场调节价,放开知名专家诊察费等131项医疗服务项目价格,由医疗机构自主定价。坚持“总量控制、结构调整、有升有降、逐步到位”的原则,降低大型设备检查检验价格,提高体现医务人员技术劳务价值的诊察、护理、手术和床位价格,共调整医疗服务项目价格近2000项。   截至目前,山东省全省参与改革的城市公立医院取消药品加成(中药饮片除外)58.01亿元,CT、磁共振等大型设备检查、检验价格总体降低20%。公立医院总体运行良好,出现了“三升五降”趋势,即业务收入、业务量、出院病人实际报销比例稳中有升,药品收入、药占比、门诊次均费用、出院次均费用、平均住院日呈下降趋势。相关的主题文章:
